Main Advantage of Ablative CO2 Laser Surgery

Instantaneous hemostasis of blood vessels smaller than O.5mm enables the use of this laser without danger to treat hemophiliacs and patients receiving anticoagulants. Sealing of lymphatic vessels theoretically reduces the risk of dissemination from potentially malignant cells. Absent or diminished post-operative pain (due to sealing of nerve endings, absence of inflammation, little swelling and little drainage). A sterile operative field reduces the risk of disseminating infection. Smaller chances of hypertrophic scars or keloids compared to conventional surgery. Fewer physical manipulations of tissues during surgery. Clearer view of operative field (no obstruction from instruments nor blood). It is often possible, in one or two operative sessions to complete a treatment that otherwise would have required several additional visits to the doctor or even hospitalization. Local anesthesia is usually sufficient. As with any surgical procedure, some complications are possible following CO2 Iaser therapy. These may include pigmentation changes (hypopigmentation or hyperpigmentation), hypertrophic scarring or keloids. However, most of these problems are reversible with time or treatments. Despite this, patients presenting very pigmented skin or a history of bad scarring could be excluded from CO2 laser treatments.

Main Advantage of Lightly Ablative Frational CO2 And Erbium Nd:YAG Laser Surgery

Instead of abrading the whole surface of the treated skin, this type of partially ablasive laser creates microscopic channels leaving an area of intact skin between each channel. During the healing process of the channels, this intact skin will close in and bring about a cutaneous resurfacing. The degree of improvement is relative to the intensity of the treatment. Multiple sessions of treatment are necessary to obtain an improvement equivalent to continuous CO2 and Erbium Nd:YAG lasers without pigment modifications (hypo or hyperpigmentation) and almost without the formation of hypertrophic or keloidal scars and with less postop inconvenience and minimal downtime.

Main Advantage of Selective Photothermolysis Lasers

These lasers provide the most up-to-date treatment available for the removal of vascular lesions, some benign pigmented lesions, tattoos and also for hair removal. Hence, they remove different types of lesions that cannot be removed through conventional surgery (on account of unaccep-table scarring). Lesions are removed with minimal pain (no anesthesia is required for most patients) with no or minimal risk of scarring (“temporary” hypo or hyperpigmentation may occur). However, many treatment sessions may be required to obtain maximal blanching of some lesions.

Let it be said that lasers have been in medical use for over sixty years and that there has not been any report of long term adversed effects or development of cancer. Some patients may have lesions that are not suitable for laser surgery, while others may present lesions where conventional surgery is often the preferred alternative.

Continuous Erbium:YAG Lasers (ConBio or Sciton)

These Erbium Nd:YAG lasers allow us to resurface the skin of the face, neck and back of hands by removing the damaged surface layer of dead skin cells. These lasers remove fine lines and superficial wrinkles and minimizes acne scars.

Continuous CO2, SilkTouch and FeatherTouch CO2 Lasers

These Lumenis conventional lasers allow us also to resurface the skin of the face. They remove fine lines and wrinkles, improve frown creases and minimize acne scars.

PICOWAY® One laser, the PicoWay is now used in our office to remove Tattoos and pigment of varied benign pigmented lesions

PICOWAY® is a new laser treatment for the eradication of Tattoos and Benign Pigmented lesions. PicoWay® The Clear Solution enables removal of multi-colored tattoos, recalcitrant tattoos and benign pigmented lesions. PicoWay’s unique mode of action is based on delivering ultrashort picosecond pulses of energy to the tissues. These bursts of energy create a photo-mechanical impact which shatters the tattoo ink or the melanin of pigmented lesions into smaller particles that are more easily eliminated by our bodily phagocytic cells.

It is possible to safely, easily and effectively remove most types of decorative, medical and traumatic tattoos without scarring. This laser is also used to treat superficial and deep pigmented lesions including everything from age spots (actinic), café au lait macules, junctional pigmented naevi and some pigmented birth marks. It allows us to reduce the number of treatment sessions necessary by comparison to the traditional lasers used up to now.

Fraxel re:pair and AcuPulse CO2 fractional lasers

These two ablative fractional lasers use a high-intensity carbon dioxide laser (CO2) to improve scars and wrinkles, to soften deep frown lines, to improve age spots, sunspots and sun-induced redness while tightening saggy skin and smoothing skin tone and texture. A laser beam creates thousands of microscopic columns surrounded by normal skin that accelerates the healing process. These fractional treatments allow your body to heal the skin much faster by stimulating one's natural healing process. This is done without the complications associated with the traditional CO2 laser. Maximum results may be obtained in one treatment session. It is done under mild sedation in addition to a local anesthetic. Since it targets deeper dermal tissues, most patients can expect a downtime of 7 to 14 days depending on the aggressiveness of the treatment. Soon after treatment, the surface layer is improved and more results will be obtained after 3 to 6 months as the deeper layers of skin will continue to heal. Results can last for years depending on your age and skin condition.

The Sciton Nd:YAG, LightSheer Diode lasers and the IPL and BBL devices

These lasers were developed for hair removal. The light emitted by these lasers is absorbed only by the pigment located in the hair follicle, that which disables the follicle and prevents the transfer of excess heat to the skin surrounding the hair follicle. The LightSheer Diode is most appropriate for dark hair in fair-skinned men or women. The Sciton Nd:YAG laser with a contact cooling device, is used for hair removal in darker skin patients. The IPL of Lumenis and the BBL from Sciton, both non laser, allow us, if indicated, to treat some patients.

The Dye Laser (Candela Vbeam Perfecta)

The Dye Laser (Candela Vbeam Perfecta): is a selective photothermolysis laser to treat cutaneous small blood vessel disease processes (telangiectasia, port-wine malformation, certain types of warts etc.). This highly selective destruction of blood vessels occurs with preservation of the perivascular area (less thermal coagulation due to its short pulse duration) and with very little action of this yellow light on the pigmented skin surface (no significant absorption by melanin). This laser, to which is attached a cooling spray ("Dynamic Cooling Device"), is also a natural complement to sclerotherapy of lower extremities to treat effectively small vessels under one millimeter in diameter.

The LaserScope Aura KTP, Cynosure Excel V and Lumenis YAG Lasers

These lasers are used to treat larger vascular lesions (facial, leg telangiectasia, peri-orbicular venulases). The Aura is used with the fiber pen for the manual treatment of linear lesions and also with a SCANNER for the treatment of larger vascular cutaneous lesions (a Cool Spot is also used when indicated). The Excel uses a gel + a contact cooling spot while with Lumenis YAG the use of a gel only is indicated.

ProFractional Erbium:YAG Laser

This Profractional laser is mainly used for superficial acne and post-traumatic scars. A laser beam creates thousands of micro channels surrounded by normal skin to accelerate the healing process while the treated areas stimulate the production of new collagen. The face, neck, chest and back of hands may be treated. A series of 1 to 4 sessions spaced 3/6 weeks depending on your needs. Minimal inconvenience: mild redness resembling a sunburn. Down-time from one to three days depending on the depth of the treated lesion. Makeup may be applied one or two days after each session of treatment. This treatment is often combined with a MicroLaserPeel.

Erbium Nd:YAG MicroLaserPeel Laser (Sciton)

This Sciton Laser is a new generation of lightly invasive Laser. The MicroLaserPeel is one of the awaited missing link between microdermabrasion and carbon dioxide laser. It is used for the treatment of superficial wrinkles. It allows a uniform and control clean vaporization selecting from 10 to 60 microns. The down-time is short, 2 to 4 days, also called the week-end wrinkle laser, according to the depth of the ablation because the MicroLaserPeel does not penetrate the 110 micron epidermal barrier. This treatment may be unique or repeated at intervals of 2 months, but results are not additive.

ClearSense Laser to treat Nail Fungus

Nail fungus is prevalent worldwide and up until now, has had very limited, ineffective, and undesirable treatment options. These options include topical antifungal creams that cannot completely eradicate the fungus if the matrix of the nail is infected, oral antifungals that are hepatotoxic, or permanent removal of the nail. ClearSense ClearToe technology heats the nail and the nail bed with laser energy resulting in a decrease of nail fungus and increase in the growth of clear nails. With the fungal growth arrested visible improvement will be seen following nail growth.

Hair Bio-Simulation Laser

Laser Bio-stimulation is a therapy that does counter genetic hair loss in men and women (androgenic alopecia). The goal of this treatment is to stop the progression of hair loss while giving the appearance of fuller, thicker and healthier hair and to increase in some patients the number of terminal hair.
